The deaths of Lizzie and Mika (in Season 4 Episode 14) is still the most upsetting/jarring episode for me thus far. Not 'gross' as such, just shocking.

I did watch the first episode last night and it was 'uncomfortable' so uncomfortable I didnt actually enjoy it... the whole episode was like wading through a boggy marsh of sadness that clung to every fibre of your being. Bastards.
